There is only one way to create a dedicated server login. Everything happens on the player pagearrow-up-right.
Once logged into the site, click on your tag right to "Manage" then "Player Page" :
In the "Dedicated Servers" section click on "Accounts" :
You can now create your dedicated server login by completing the requested fields:
